Submitting a Research Proposal

In order to submit a research proposal to the Research Ethics Committee of the University of Sao Paulo Medical School General Hospital (HCFMUSP) one must necessarily follow the instructions contained in this manual, which aims at informing researchers about which forms and documents are needed to get the ethical approval of their research.

Instructions for preparing the Informed Consent Form

For research protocols to be submitted, the research proposal must include the following documents:

Cover Sheet of Plataforma Brasil signed by the responsible researcher (3 printed copies);
Cover Sheet of the HCFMUSP Research Management System signed by the responsible researcher (3 printed copies);
Full Research Protocol (1 printed version + 1 electronic file);
Informed Consent Form (3 printed copies);
Reference Forms (3 printed copies):

Form.003 - Commitment Agreement for the Responsible Researcher 
Form.004 - Commitment Agreement for Additional Researchers
Form.005 - Consent Form for Institute's Departments
