In order to submit a research proposal to the Research Ethics Committee of the University of Sao Paulo Medical School Clinical Hospital (HCFMUSP) one must follow some instructions, which aims at informing researchers about which forms and documents are needed to get the ethical approval of their research.
Any participation of researchers from other organizations or Departments and Institutes of the University of Sao Paulo (USP) are subject to a collaborative agreement with the leading researcher of the intended research area, and, ultimately, to approval by the Board of Directors of the Institute of Physical Medicine and Rehabilitation (IMREA HCFMUSP).(It must be filled in portuguese in by a researcher who necessarialy should have a link with HCFMUSP)
Instructions for preparing the Informed Consent Form
For research protocols to be submitted, the research proposal must include the following documents:
Cover Sheet of Plataforma Brasil signed by the responsible researcher (3 printed copies);
Cover Sheet of the HCFMUSP Research Management System signed by the responsible researcher (3 printed copies);
Full Research Protocol (1 printed version + 1 electronic file);
Informed Consent Form (3 printed copies);
